ARE YOU INTERESTED in having a personal health coach to help you set healthy lifestyle goals? With HealthScore, you have an opportunity to have a personal health coach who can help you by working with you on exercise that you can do from home; help you monitor symptoms; address other areas of your experience with cancer that may be important to you (for example, sleep, nutrition, quality of life, overall goals); and navigate you to resources on the web, locally, or at the cancer center that can provide you with supportive care. If you have cancer or are a survivor of cancer, you may be able to participate.
What is the purpose of the HealthScore program?
- To deliver 6 months of 1:1 health coaching to individuals with cancer and survivors
- To improve your self-confidence and how you feel as a person and not just a patient in your day-to-day life.
What does the HealthScore program include?
- Exercise goal setting and monitoring
- Symptom monitoring and management
- Support for goals that you identify as important to you (nutrition, sleep, quality of life, and others)
- Support for your emotional and social needs
What will you receive if you participate in the HealthScore program?
- A FitBit to help you track your physical activity
- Access to a symptom and other cancer needs reporting web-based platform
- Access to a website where you can see your data and check out resources
- Weekly calls from a trained coach
